  • Please be sure to sign all of the consent forms, add your insurance information, and upload a picture of your driver’s license prior to your appointment. This ensures that we have all the necessary information to provide you with the best care possible. 

Health Questionnaire

  • Filling out the health questionnaire prior to your appointment is crucial. It helps our medical team understand your health status and prepare for your consultation effectively.

  • We may cancel your appointment if the necessary forms are not received prior to your telemedicine appointment. Your cooperation is greatly appreciated to ensure a smooth and efficient service.

Appointment Cancellation Policy

As stated above, it is crucial that all required forms are submitted prior to your telemedicine or in-person appointment. Please make sure to sign all consent forms, add your insurance information, and upload a picture of your driver’s license. Additionally, complete the health questionnaire before your scheduled time. Failure to submit these documents may result in the cancellation of your appointment. We appreciate your cooperation and understanding.

Please note: You may be charged a NO SHOW FEE if you do not cancel your appointment within 24 hours.
